Inadvertent Overdose of Low-Molecular-Weight Heparin in an Elderly Patient with Deep Vein Thrombosis and Acute Kidney Injury.

چکیده
Low-molecular-weight heparin (LMWH) is commonly used to treat deep vein thrombosis. The dose is usually based on the patient’s body weight, and for some preparations the manufacturer recommends a daily maximum dose. Unlike the situation for unfractionated heparin, no effective antidote is available for LMWH overdose. Protamine sulphate neutralizes the anticoagulant effect of unfractionated heparin, but it is only partially effective against LMWH. Protamine sulphate is composed of polypeptides that bind heparin to form a stable, inactive complex that inhibits the anti–factor IIa effect of LMWH,1 but the effect on anti–factor Xa is only partial,2,3 reaching up to 74%.4 Despite the frequent use of LMWH, there are few descriptions of management strategies for LMWH overdoses. Furthermore, most case reports of LMWH overdose have involved patients who were relatively young.1-3,5-7 To the authors’ knowledge, the current article is the first report of LMWH overdose in a patient over 80 years of age.

A Rare Presentation of Arterial Thrombosis after Venous Thrombosis in a Patient with Gastric Cancer
Background: Venous thromboembolism, which comprises deep vein thrombosis and pulmonary embolism and is a significant cause of morbidity and mortality among patients with cancer. The aim of the present study was to report a rare presentation of arterial thrombosis after venous thrombosis in a patient with gastric cancer. Report of the Case: A 45 year-old female patient with locally advanced gas...
متن کاملLong-term low-molecular-weight heparin and the post-thrombotic syndrome: a systematic review.
OBJECTIVE Post-thrombotic syndrome causes considerable morbidity. The Home-LITE study showed a lower incidence of post-thrombotic syndrome and venous ulcers after 3 months of treating deep vein thrombosis with the low-molecular-weight heparin tinzaparin versus oral anticoagulation. This systematic review examined whether long-term treatment of deep vein thrombosis using low-molecular-weight hep...
متن کاملTwo cases of jugular vein thrombosis in severely burned patients
Here we present two cases of jugular vein thrombosis in burn patients, with diagnosis, risk factor analysis, and treatment approaches. Severely burned patients have high risk of deep vein thrombosis occurrence due to multiple surgeries. The deep vein catheter should be carefully performed. Once deep vein thrombosis is detected, a wide ultrasonography helps to find other thrombosis sites. During...
متن کاملEconomic evaluation of outpatient treatment with low-molecular-weight heparin for proximal vein thrombosis.
BACKGROUND The safety and efficacy of taking low-molecular-weight heparin at home was previously demonstrated in a clinical trial in which patients with acute proximal deep vein thrombosis were randomized to receive either intravenous standard heparin in the hospital or subcutaneous low-molecular-weight heparin administered primarily at home. Treatment in the home has the potential to substanti...
